I remember that. That's historical. Then, like Jacko, it busted out and went solo.
It went from hamburger sandwich... to hamburger sandwich with cheese... to hamburger and cheeseburger, whereby the latter somehow freed "burger" to stand on its own as an entire category separate from sandwiches. Hence the title of this thread. I say, I'm a burgher, too. Aren't you? I feel a musical coming on....
